Confirmation Testing Request

Thank you for choosing Virant Diagnostics' U-Catch 12-Panel Urine Drug Screening Cup. Send your sample back to us for quantitative confirmation testing if you have any positive screens or if you simply want definitive results. To request confirmation testing, please fill out the form below:

Once you have completed registration and payment above, please follow the sample preparation instructions below:

  1. Place the completed label with your name, date of birth, screening results, and collection date and time on the biohazard bag. 
  2. Place the urine cup in the biohazard bag. Ensure that both the cup and the bag are properly sealed.
  3. Put together the included shipping box and place the biohazard bag with the urine cup inside.
  4. Keep the shipping box with the sample in the refrigerator until you are ready to ship it.
  5. Ship the sample with your desired carrier. NOTE: Virant Diagnostics is not responsible for any lost or damaged samples during shipping.
  6. Once the confirmation test is completed, you will be notified by email or text to access your results through our secure patient portal.
