Confirmation Testing Request Form. All fields are required.

Once you submit the registration above, you will receive an email with further instructions. In the meantime, please follow the sample storage instructions below.

  1. Place the completed test requisition card into the assembled shipping box. 
  2. Place the urine cup in the biohazard bag and refrigerate until ready to ship.  
  3. When ready for shipping, insert the biohazard bag (containing the capped urine cup) into the shipping box with the test requisition card.
  4. While refrigerating, complete payment for confirmation testing. (Payment options with further instructions will be provided in the email you received by clicking “Submit Registration” above.)
